Hospice care is most often provided for the patient in their home – wherever they call home – with familiar surroundings. This helps to reduce anxiety and allows the patient and their family or caregiver to focus on their time together.
What to expect from hospice care at home
Your Care Team members will visit as needed and teach caregivers the skills necessary to care for the patient when we are not there.
Your hospice team communicates with your physician and the hospice physician to discuss medical history, current physical symptoms, goals of care, preferences for end-of-life care, and life expectancy.
Other components of hospice care includes:
- Necessary home medical supplies and equipment
- Medications related to the life-limiting diagnosis
- Assistance with the activities of daily living
- Emotional support and companionship
- Education on how to manage symptoms
- Instructions for emergencies
- Support for caregivers and loved-ones during and after this journey
Where is care provided?
Care is provided wherever the patient calls home. It may be a private residence, nursing home, assisted living, home plus or independent living community, to name a new.
Hospice care considerations
The patient and family’s wishes are at the heart of the care plan.
The hospice team listens to input from the patient, family, caregiver(s), physicians, and reviews medical evaluation(s) from the registered nurse, to develop the personalized care plan. The plan is assessed regularly and revised based on the patient’s needs and goals.
