Hospice resources for healthcare professionals
As a healthcare partner, we value your referrals.
We are committed to providing a wide range of hospice care resources and serving to improve the quality of life for your patients with an advanced illness. As a community leader in hospice care since 1983, Harry Hynes Hospice has cared for numerous complex patients and managed thousands of health care transitions.
We have been a community leader in Hospice Care for 40 years – and still counting!
Our reputation is well-known for our deep understanding of end-of-life needs. We have been there many times before, and we will be there for your patient with compassion and understanding.
During the admission process, our team will work with you as well as the patient to determine what their goals are. And we will continue to consult with you regarding how best to meet the needs of your patient as their condition transitions.
Benefits of hospice care
Hospice care provides symptom management, pain relief, patient comfort and focuses on quality of life.
In some cases, trips to the hospital can be avoided with high-quality, coordinated hospice care. Patients who have a team of professionals they can call with questions and who are stabilized at home may have less need to visit the emergency room or hospital.
Senior living facilities that acknowledge hospice care have experienced reduced hospital admissions and re-admissions.
They have also experienced higher quality ratings, patient satisfaction scores, and improved quality of life near the end of life for patients and their families.
Sometimes, with the extra care and other benefits of hospice care, patients’ quality of life can improve. In fact, research shows patients on hospice can live longer than patients who do not receive hospice care.
The benefits of hospice care extend to family members and caregivers as well. Hospice provides education, guidance and a listening ear to help loved ones with the difficult work of caregiving. Volunteers may lend a hand so the caregiver can focus on caregiving.
Hospice aides help with bathing, dressing and other everyday tasks. Respite care may also be available to give caregivers some time off while their loved one is cared for in an inpatient unit or skilled nursing facility for up to five days. Hospice also provides bereavement support to help loved ones work through grief and other feelings.
